The Composite Bonding Company –
Late Notice Cancellations and Failure to Attend Policy
We aim to provide all our patients with the best possible service and to achieve this we need your cooperation.
If you are unable to keep your appointment, please make every effort to cancel it well in advance so that it may be offered
to someone else. Non-attendance and cancellations at short notice without a valid reason deprives other patients of our
services.
Patient’s will need to understand and agree to the following terms:
- The Composite Bonding Company –
Fail To Attend and Late Cancellations Policy Agreement
I, the patient, understand that the session booked is a private service and all charges are payable in
advance of my appointment.
The Composite Bonding Company has made a services recommendation and has discussed with me the
initial assessment and the costing’s associated with the first appointment. Should I require further
appointments, I understand the Clinician will discuss a treatment plan and costs in detail.
I understand and agree to the following terms:
Any appointment requires a 50% deposit of its total cost in order to book it.
If I need to cancel or change any appointment then I am required to do so with at least 24 hours’ notice.
We reserve the right to retain this fee in the event of late cancellation (less than 24 hours) OR if you fail to
attend.
• If I arrive late for my appointment then I will only be seen for the remaining time allocated. This may then
have an effect on the amount of work can be completed.
• All payments will be made in advance of an appointment in line with The Composite Bonding Company’s
policy.
• If I late cancel (less than 24hrs notice) or If I fail to attend then my 50% deposit for that appointment will
be consumed to compensate for lost ‘surgery time’ and business income.
• If I contact the practice and give at least 24 hours’ notice that I cannot attend my appointment, then the
50% will be transferred to my next appointment.
• If I have two late cancellations or fail to attend in a rolling 12 month period it may result in no further
appointments with the practice.
